BEFORE YOUR APPOINTMENT

• The intake form must be filled out and submitted prior to your first appointment. It will enable me to customize your appointment to your specific needs. If there is a question you cannot answer, simply put N/A. You will not need to fill out new forms for future appointments unless any medications or allergies change.

Cancelations

• Your appointments and convenience are of high importance to us. We realize that at times unexpected circumstances may require schedule adjustments. If you need to cancel an appointment, we respectfully request that you provide at least 24 hours notice. This will allow us the time to book you a new appointment and fill the newly available slot. Please keep in mind that we are a small family-owned business.

LATE ARRIVALS

Should you arrive more than 10 minutes late for your appointment, we cannot guarantee that we will provide the service. We will try our best to reschedule your appointment for another time that suits you.
